If you have submitted a request under the Personal Health Information Act (PHIA) to a custodian for access to a record or for correction of your personal health information and you are not satisfied with the response, you may file a complaint with the Office of the Information and Privacy Commissioner (OIPC).

If you have received a fee estimate or a custodian has charged a fee in relation to an access request and you are not satisfied with it, you may file a complaint with OIPC.

If you believe that a custodian improperly collected, used, or disclosed personal health information, you may file a privacy complaint with OIPC under PHIA.

Custodians must report all material privacy breaches to our Office. However, we encourage custodians to report all privacy breaches.
