How to Request Records

Access to Information and Protection of Privacy Act, 2015

Under the access provisions, individuals or organizations (applicants) who wish to obtain access to records may do so by completing the appropriate form and forwarding it to the public body that has custody or control of the information being sought.

If you are making your request to a department of the Government of Newfoundland and Labrador, you can click on the box Instructions for Making a Request to a Government Department and it will take you to their online request form.

When making an access request to a public body other than a government department, the request must be made in writing. A copy of this form can be found by clicking on the box Access to Information Request Form. Once completed the form must be sent to the ATIPP Coordinator of the public body. Personal Health Information Act

The Personal Health Information Act (PHIA) gives individuals the right to access their personal health information in a custodian’s custody or control, and the right to request correction of their personal health information where it is factually inaccurate.

You must make your request for access to personal health information to the appropriate custodian, preferably in writing. However, custodians may also accept oral requests. Requests should contain the individual’s name, date of birth, MCP number or other unique identifier, and address. They should also contain information regarding the scope of the request to allow for record retrieval with reasonable effort, such as the dates the custodian would have collected, used, or disclosed the personal health information.  Some custodians have developed forms for such requests and it is best practice to ask about such forms prior to making a request.
