You may email, drop off, mail, or fax your complaint to our Office. We must have an effective and timely means of communication with complainants. Ideally, we can achieve this through email or telephone, so please provide us with your contact information. If you are unable to communicate with this Office via email or telephone, we will make every effort to investigate your matter to the fullest extent. However, time constraints may hinder your participation. If you have questions about completing these forms, please contact our Office.

Please be aware that when using certain browser plugins the PDF files below may not open properly. Please ensure you have updated to the latest version of Acrobat® Reader.

Please note: Some browsers handle pdf forms differently. If you are having difficulty filling out a form in the browser window, please follow the steps below which explain how to save the file to your computer.
  1. Right click on the link to the form.
  2. Select “Save target as” or “Save link as.”
  3. Select a location on your computer to save the file.
  4. Open the file from the saved location. If you cannot open the file, download Acrobat Reader for free.
  5. Complete the form.
  6. Save the form to your computer.
